I Know This Much Is True was filmed in New York State in the cities of Poughkeepsie, Wappingers Falls, and Kingston. Filming also took place at the Umbra of Newburgh soundstage in New York.

In Poughkeepsie, locations like the Vassar Brothers Medical Centre, Vassar College, Seasons Fishkill Restaurant, 9 Mall Plaza, and All Sports Health and Fitness were used for various scenes.

According to Cinemaholic , Lamb wanted the show to be filmed in Eastern Connecticut, and he took Ruffalo and director Derek Cianfrance on a tour of various locations in Mansfield, where he currently lives.
